793 RECURRENT PERICARDITIS AND COVID-19

Abstract Background Anakinra, an interleukin 1β recombinant receptor antagonist, could be used to reduce corticosteroid-dosage in colchicine-resistant pericarditis. Recently anakinra has been approved treat patients presenting covid-19 related pneumonia requiring supplementary oxygen at risk of developing ARDS (identified by the finding sieric suPAR least 6 ng/mL.) Clinical case A 86-years-old woman with history hypertensive heart disease, diabetes mellitus and hyperthyroidism. Echocardiography showed pericardial effusion signs early-stage cardiac tamponade pericardiocentesis. Post-procedural AF occurred. Considering potential high bleeding caused dose NSAIDs, set up medical treatment colchicine corticosteroid. During hospitalization patient tested positive for SARS-CoV-2. At same time fluid cultures were negative infectious non-infectious etiologies. Despite persisting chest pain (sharp pleuritic, improved sitting leaning forward), elevated serum CRP a pericardiocentesis residual right ventricle effusion, there fast improvement clinical picture laboratory findings after Anakinra. Conclusions IL-1 inhibitors (Anakinra) exploited COVID-19-associated mortality symptoms refractory first lines conventional pericarditis therapy.
